You will receive an email with your appointment time and a Zoom link to join the call. Please download and install the Zoom app for your device using the links at the bottom of this page for your device in advance of your appointment.

Please do not submit multiple requests. We will get back to you with your appointment information as soon as possible, Monday through Friday between the hours of 8:30 am and 4:30 pm.
