One in three American adults are at risk for developing kidney disease and may not even know it. You might have no symptoms or they may not be noticeable. Please take this simple test to find out whether you’re at risk so that you can take action.

If you scored 4 or more points: You have a 1 in 5 chance of having chronic kidney disease. A simple blood and urine test can be checked to find out. Please speak to us or ask your primary care provider.
If you scored 0-3 points: You probably do not have kidney disease now, but at least once a year, you should re-take this survey and check with your primary care provider about the health of your kidneys.
